Patient should be aware that every surgical operation has risk of complications, and only the surgeon who does not operate has no complications. A knowledge of relevant complications and sequences is essential to enlighten the patient so that an informed decision can be made, for reducing the incidence of such complications, for minimizing the gravity of an impending complication, and for treating a complication once it has occurred.
Plastics surgeons from Villa Medica clinics analyzed range of complications found in different clinics worldwide and presented overview in series of articles. The first article is about complications in rhinoplasty.
Rhinoplasty is arguably the most demanding of all facial surgical operations. It differs from other facial operations in requirements it poses to plastic surgeon.
First of all, your doctor should have deep knowledge of an art and science. Rhinoplasty cases are too specific and require a careful estimation of the deformity preoperatively and a clear understanding which techniques to use for correction. When I was going to make rhinoplasty, my doctor simply explained: it’s easy for you, because you just sleep and wakeup, but it’s really difficult for surgeon.
Secondly, complication of a rhinoplasty are really difficult to hide or mask, so if you’ll got nose malfunction, it will be visible to others and may require additional plastic surgery operation.
Complications of rhinoplasty may be divided into 4 basic categories (depending on timeframe): intraoperative (during the operation), immediate postoperative (in the recovery period), early postoperative (6 month after the operation), late postoperative (more than 6 months after rhinoplasty). According to statistics, the complication rate for nasal surgery varies from 4 to 18.8%. In those numbers, skin and associated soft tissue complications occur in up to 10% of cases, severe systemic or life-threatening complications occur in 1.7-5% and intracranial complications are rare.
The first example of depicting a rhinoplasty complication be seen below.
You can see here problems caused with failure to appreciate the role of the lower lateral cartilages in tip and alar support. Transection of these cartilages near the domes and excessive excision may cause collapse of the alae and a pinched tip. Along with a pinched tip and saddling, alar collapse leading to nasal airway embarrassment was noted.

The second example shows problems, which may be caused by failure to appreciate the role of the lower lateral cartilages in tip and alar support. Transection of these cartilages near the domes and excessive excision may cause collapse of the alae and a pinched tip. In this case, the cartilaginous dorsum is also over excised.

The worst rhinoplasty was made to Michael Funk by Dr. Jack P. Gunter from Dallas, Texas.

As you see, excessive undermining, injudicious cautery use, and overzealous skin thinning may lead to skin necrosis. This also may cause a impaired blood supply and infection.
Patient explains the disaster as follows:
It is my opinion that the nose was built up with WAY TOO MUCH RIB and when he closed it back it was like trying to put a car cover that used to fit on a small car onto a now much longer and larger car....When they draped this skin back over the nose and stitched it between the nostrils, The skin could just not handle the stress of the new framework and started dying "ON THE STITCH LINE".....It did not start on the tip in the front of the nose,But underneath where it was stitched up horizontally BETWEEN THE NOSTRILS.When touching this area it felt stressed and very, very thin.Like touching bone! It was milky and black right after surgery.
This article is the least aesthetic I ever made. However, I believe, it essential for customer to understand importance of careful selection of a surgeon. Otherwise, results can me unpredictable, like in samples I showed above.
